CASE STUDY:

 

Claim:

The claimant was receiving disability benefits while convalescing at home after a work related injury.

 

Injury:

The claimant’s injuries consisted of pain in his back, neck and shoulders after being knocked down by a moving forklift.   

 

Investigation:

Our investigation revealed the subject was employed at a second job, working as a landscaper, “under the table” while receiving disability benefits.

 

On this particular day, the claimant serviced 13 yards. Not once during the 26 times of loading or unloading the mower did he use the functional hydraulic lift gate on the truck.
